Report It!

This secure, online report form serves multiple purposes. Selecting the appropriate incident/concern type will update the report form specific to your concern. Each report goes only to the appropriate department or college and will be addressed according to each department or college/s respective procedures.

Bellevue University believes that informal resolution helps in the development of the student. Students and employees should seek informal resolution with the department or academic college before submitting a ticket. The exception to this is any Sexual Harassment/Discrimination concern; if this is your concern, please continue to the appropriate form.

Please be aware that anonymous reporting may not be available and/or reports may not be able to be addressed adequately if full information or names of individuals involved are not provided. Some types of reports may require limited disclosure of the identity of those involved (including an alleged victim) by law or for investigative purposes. In addition, individuals making reports may not be entitled to information on the progress or resolution of reports due to privacy requirements under law or policy.

Academic Misconduct violations include academic dishonesty, cheating, and other failures to act with academic integrity.

If you report "Academic Misconduct," the report is submitted to the Dean of Students Office to record in the Academic Misconduct Repository. Incidents will be handled according to the Bellevue University Academic Misconduct Policy and Student Code of Conduct.

Code of Conduct violations include any violation of the Student Code of Conduct.

If you report "Code of Conduct Violation," the report is submitted to the Dean of Students Office to address according to the Code of Conduct.

Report It

Select this option for reports, concerns, complaints, or other feedback that does not fit any other category. This option should also be selected for reports of hazing, which will be processed under the Anti-Hazing Policy.

If you report "Complaint/Feedback," the report will be routed to the appropriate college or department. The report will be addressed according to each department or college's procedures. Before submitting this form, you should attempt to get a resolution at the college or department level. If no resolution has been reached, please move forward with submitting this form.

Select this option to submit a counseling services referral for a current BU student.

Counseling sessions are free of charge, up to three sessions per student, on the BU main campus only.

Report It

If you report "Grade Appeal," all information submitted will be compiled and sent to the Dean of the appropriate college for follow-up. Before submitting this, you should first attempt to get a resolution with your instructor. If no resolution is reached, please move forward with submitting this form. Please note that this form cannot be submitted until the conclusion of the course in question. One or more of the following criteria is required for a Grade Appeal.

  • The Instructor violated the terms of the syllabus (You must attach the syllabus and indicate which terms were violated and how they were violated).
  • The instructor made an error in calculating or recording the grade (You must explain the miscalculation or recording of the grade).

The Instructor violated University policy (You must explain the nature of the violation).

  • The Instructor applied an inconsistent grading standard across students (You must provide evidence of the inconsistency).
  • The Instructor violated a written agreement (e.g., email agreement) with the student (You must provide the written agreement and indicate how it was violated).
  • The instructor made an error in determining an incident of academic dishonesty (You must provide evidence that establishes the determination was in error).
  • Per the Grade Appeal policy, you have 30 days from the time your final grade posts in BRUIN to submit a grade appeal.

Harassment/Discrimination reports are processed under either the Title IX Policy (for reports of sex-based harassment/discrimination) or the Non-Discrimination and Harassment Policy (for reports based on race, color, religion, national origin, age, disability, genetic information, military obligations, or status in any other group protected by local, state, or federal law). Please identify the specific basis of alleged discrimination or harassment in the report.
